Strengthening Pathways & Engagement with Primary Care

Inclusion Matters Wirral is keen to strengthen our relationship with Primary Care, community groups and other partnership agencies and would be pleased to come to practice meetings and share information, provide training or to help problem solve.

Step 1 liaison Role

Step 1 of the 'Stepped Care Model' is held within Primary Care GP service settings. This involves monitoring patients who fall below the threshold for psychological therapy and hazardous drinking patterns or who initially choose not to engage with psychological interventions. 

As the lead for Step 1 within Inclusion Matters Wirral my role is to provide a gateway of collaborative working and communication between GP’s and a referral into our service.

We provide a description of the stepped care approach for psychological and alcohol IAPT services, to inform how psychological therapies can help people and promote a shared pathway of care for patients being referred for therapy.

The benefits of this joint pathway, enables an engaging relationship between IML and GP’s. It maintains a closer monitoring of referral numbers and numbers of patients ‘opting in’ to the service as well as waiting times and service developments. 

Step 1 also involves the roles of ‘Locality Leads’ who have a closer liaising role with specific GP’s and practices within their cluster area. Their role is to enhance support and education to GP teams and improve the information required for appropriate referrals into our service.

Locality leads aim to encourage the process of ‘active monitoring’ with GP’s and to liaise with other neighbourhood and locality based services for GP’s to signpost to.
